45-49 Year Old Health Checks
At this stage of your life, any recommendations made by your GP based on a health check can impact your health and wellbeing later in life. Statistically, between the ages of 45-49 years is when you might have a higher chance of showing early signs of a chronic disease. This will involve a 30-minute consultation with the GP so if you fit within this age bracket and are interested in the health check, ensure to book a longer appointment.
Over 75 Year Old Health Checks
Patients over 75 years are eligible once a year for a thorough health check which assesses your health from a variety or parameters including physical, psychological, and social health. It can help to identify any increased needs you may have before things get on top of you. This will involve a 30-minute consultation with the GP so if you fit within this age bracket and are interested in the health check, ensure to book a longer appointment.
Healthy Heart Checks
Your GP can conduct regular heart health checks to know what your risk of a heart attack or stroke may be. This can allow you to make important changes to lifestyle and/or medication to reduce this risk and stay well for longer. Patients who are aged 45 years and over are eligible for a heart health check or those patients 30 years and over who are of Aboriginal or Torres Strait Islander descent.
Aboriginal or Torres Strait Islander Health Checks
The aim of this health assessment is to help ensure that Aboriginal and Torres Strait Islander people receive health care that is matched to their needs. It is designed to encourage regular assessments for early detection and intervention for treatable conditions to reduce the likelihood of poor health. This health assessment is available every 9 months and for all ages for eligible patients. You will need a longer appointment for this assessment.
Our doctors are experts in managing chronic diseases which is any condition that may last a year or more and requires medical attention or limits your activities of daily living. Examples include, but are not limited to, heart disease, diabetes and cancer. Each year, a Chronic Disease Management Plan can be completed by your GP to detail how you will manage the disease as well as what specialist and allied health professionals you may have involved in your care. If you are wanting to discuss this with your GP, you may need a longer appointment to go through everything thoroughly.
